EMERGENCY ACTION PLAN and player medical forms
he STHF Emergency Action Plan (EAP) is designed to equip coaches with the knowledge and tools to respond calmly, quickly, and effectively if a player is seriously injured during practice or competition. It ensures that coaches can create a detailed, location-specific plan that includes all necessary information for emergency response, including medical contacts, procedures, and communication protocols.
Coaches will submit their EAP upon registration and will have all players fill out Medical Forms that will be carried or available at all activities in a medical kit (practice, tournaments, league games)
